How to Apply

There are 3 steps to apply to become a member:

See if you are eligible

The WTC Health Program serves four groups of people affected by the 9/11 attacks:

  • FDNY Responders,
  • WTC General Responders,
  • WTC Survivors (lived, worked, or went to school in NYC Disaster Area or were present in the dust or dust cloud), and
  • Pentagon/Shanksville Responders

Eligibility criteria for these groups are defined by the James Zadroga 9/11 Health and Compensation Act of 2010, and National Defense Authorization Act for Fiscal Year 2024 expanded enrollment eligibility for Pentagon and Shanksville responders.

Review supporting documentation requirements

Each eligible group has different supporting documentation requirements. You must include supporting documentation that details the activity, location, time period, and hours requirements that are outlined in the eligible groups criteria.

Complete your application

Once you’ve determined your eligibility and gathered your documentation, the next step is to apply. You can apply online or submit a paper application by mail or fax. The paper application is also available in  Español, Polski, and 中文.
