How to Use V-safe

At a glance

  • Sign up for V-safe within 42 days of getting a COVID-19 or RSV vaccine.
  • Complete V-safe's check-ins. A check-in takes less than two minutes to complete.
  • You can sign up for V-safe and complete check-ins for someone else if you are their parent or caregiver.
  • If you previously signed up for a V-safe account, you will need to create a new account to share your experience with the currently monitored vaccines (COVID-19 or RSV vaccines).

Signing up for V-safe

  1. Using your computer, tablet or smartphone, visit the V-safe sign-up page.
    1. You can also scan the QR code located on the information sheet using your tablet or smartphone camera.
  2. Fill in the required fields to verify you do not have an existing account.
    1. If V-safe does not detect an existing account, select Sign up.
    2. If V-safe detects an existing account, select Log in.
  3. Answer the account sign-up questions including demographics and contact information. Select Submit.
    1. You must sign up for an account for yourself before participating in V-safe for someone else.
  4. V-safe will send you a verification code. Enter the verification code and select Submit. A message will appear on the screen confirming that your account has been successfully created.
  5. V-safe will send you a follow-up link. Click and follow the instructions to activate your account.
  6. Follow the next series of instructions to add a new vaccine to your V-safe account.

Error messages when trying to sign up‎

Carefully check the information you enter, especially the verification code V-safe sends you. If all your information is correct, try closing the browser window and starting over. If you need additional assistance, email the help desk at vsafesupport@cdc.gov.

Enrolling in a vaccine you received

  1. Log in to V-safe.
  2. Select Enroll.
  3. Answer initial questions about the vaccine(s) you received. Take time to review and confirm that the information is correct before selecting Submit. This will be the only opportunity to modify your entry.
  4. If you enrolled before 12 p.m. local time, V-safe will send you an email or text to start your initial check-in around 12 p.m. the same day. If you enrolled after 12 p.m., V-safe will send you a link to start your initial check-in immediately — just follow the instructions.
    1. Check-in links expire. Try to complete your check-in promptly after receiving your reminder.

Adding a dependent in V-safe

You will be able to add a dependent under "My Account" once you have signed up for V-safe. It is helpful to have your dependent's vaccination information available to enter details about which vaccine(s) they received and when. If this information is not available, please contact your vaccination provider.

  1. Log in to V-safe.
  2. Select Add Dependent (located to the right of or below your name in the upper left corner).
  3. Enter your dependent's demographic information and select Submit.
  4. You will now see individual blue buttons for each dependent at the top of the screen on the Home tab.
    1. You can also see your dependent(s) by selecting the My Account tab.

Enrolling a dependent in a vaccine they received

  1. Log in to V-safe.
  2. Select your dependent's name (located to the right of or below your name in the upper left corner).
  3. Select Enroll.
  4. Answer initial questions about the vaccine(s) they received. Take time to review and confirm that the information is correct before selecting Submit. This will be the only opportunity to modify your entry.
  5. If you enrolled your dependent before 12 p.m. local time, V-safe will send you an email or text to start their initial check-in around 12 p.m. the same day. If you enrolled your dependent after 12 p.m., V-safe will send you a link to start their initial check-in immediately — just follow the instructions.
    1. Check-in links expire. Try to complete your check-in promptly after receiving your reminder.

Note: To add a follow-up dose on behalf of your dependent, select your dependent's name in the upper left corner, and follow the same instructions as below.

Adding follow-up vaccine doses

See all vaccinations that you have enrolled‎

Select the Vaccination History tab at the very top of your V-safe homepage. On some mobile views, you must first select the More dropdown button to access this page.
  1. Log in to V-safe.
  2. In the "Enrolled Activities for [Name]" table, select the vaccine you received.
  3. Select Add dose.
  4. Answer initial questions about the additional vaccine dose you received. Take time to review and confirm that the information is correct before selecting Submit. This will be the only opportunity to modify your entry.

Completing V-safe check-ins

  1. V-safe will send reminders to complete your (or your dependent's) regular check-ins to your preferred method of contact.
    1. Check-in links expire. Try to complete your check-in promptly after receiving your reminder. Once a check-in has expired, it is no longer available for completion.
  2. Click and follow instructions to answer questions (about you or your dependent) when you're ready. Check-ins should take less than two minutes to complete and must be completed within the United States and its territories.
  3. Take time to review and confirm that your survey responses are correct before selecting Submit. This will be the only opportunity to modify your response.
  4. V-safe will send check-in reminders for up to six weeks (or more, for maternal RSV vaccine recipients). You can opt out at any time.

Select the Check-ins tab at the very top of your V-safe homepage to review your (or your dependent's) previous responses.

Depending on the answers you provide during the check-ins, you may receive a message via your preferred method of contact asking you to give more information about your vaccination experience and side effects by completing a VAERS report.
