The first section is for indicating data on the number and percentage of adolescents in the clinic who are vaccinated for HPV, as compared with 2 other adolescent vaccines. The second is to help set a goal to improve HPV coverage and indicate progress at 3 and 6 months. The third is a recommendation to parents to have their child vaccinated, starting at age 11 years.
