Timeline from July 2014 through December 2015 begins with the National Cancer Institute’s request for proposal in July 2014, initiation of media scan in August 2014, funding award in September 2014, hiring of coordinator in November 2014, monthly key informant interviews beginning in December 2014, national planning meeting in January 2015, joining National Cancer Institute’s Research to Reality HPV Vaccination Learning Community in February 2015, survey finalized and piloted in July 2015, completion of media scan in August 2015, provider survey in August 2015, provider interviews in September 2015, and the concluding meeting with NCI in November 2015. The timeline also shows that we made 6 informational presentations, had 3 meetings of Kentucky’s HPV team, and the various stages of development (steps 1–3), implementation (steps 4 and 5), evaluation (step 6), and dissemination (step 7).
