The Chronic Care Model and Diabetes Management in US Primary Care Settings: A Systematic Review
The initial search produced 155 articles: 17 articles from the Cochrane Database of Systematic Reviews, 79 articles from CINHAHL via EBSCOhost, and 59 articles from Health Source: Nursing/Academic Edition via EBSCOhost. The articles were reviewed in 3 steps. In Step 1, abstracts were reviewed. As a result, 76 articles were included; 79 were excluded for the following reasons: 11 were not conducted in the United States; 33 were not related to the scope of the study; 16 were literature reviews or meta-analyses; 7 were editorials or commentaries; 3 did not have enough information to assess the study; and 9 were duplications of excluded articles. In Step 2, articles were reviewed for content and relevance. As a result, 43 articles were included, and 33 were excluded for the following reasons: 13 were not related to the scope of the study (eg, diabetes was not the primary focus, analyzed current practices without implementing the CCM, reported secondary data, had a financial focus), and 20 were duplications of included articles. In Step 3, the articles were analyzed according to application of the CCM. As a result, 27 articles were excluded because they were not related to the scope of the study (eg, use of CCM not clearly described, focus on instrument development rather than CCM application), and 16 articles were included. These 16 articles were peer-reviewed, conducted in the United States, and published in English, clearly delineated use of CCM, took place in primary care settings, and represented original research.
