What to know

Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Vermont.

General Information

Vermont's IIS is the Vermont Immunization Registry (IMR), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

The jurisdiction is authorized by law to operate IMR. For more information, please see 18 Vermont Statutes § 1129(b).

Child Consent to Participate

For children, IMR requires mandatory inclusion by operational policy. Children are included in the IIS with no possibility for their parent(s)/guardian(s) to opt them out.

Adult Consent to Participate

For adults, IMR requires IMR requires mandatory inclusion by operational policy. Adults are included in the IIS with no possibility to opt out.

Provider Reporting

Vermont does have a mandate for vaccination providers to report immunizations to IMR. The reporting mandate:

  • Mandates universal reporting by law.

Data Sharing

Vermont does have a policy that regulates sharing data from IMR with other public health entities. The data sharing policy:

  • Requires a data use/sharing agreement or memorandum of understanding,
  • Requires protecting the data's confidentiality, and
  • Allows vaccine recipients to request to limit the sharing of their data that are stored in the IIS.
