What to know

Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in South Carolina.

General Information

South Carolina's IIS is the South Carolina Immunization Registry (SCI Registry), also referred to as the Statewide Immunization Online Network (SIMON). SIMON includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

Child Consent to Participate

For children, SIMON requires mandatory inclusion by law. Children are included in the IIS with no possibility for their parent(s)/guardian(s) to opt them out.

For adults, SIMON requires mandatory inclusion by law. Adults are included in the IIS with no possibility to opt out.

South Carolina does have a mandate for vaccination providers to report immunizations to SIMON. The provider reporting policy:

South Carolina does have a policy that regulates sharing data from SIMON with other public health entities. The data sharing policy:

  • Allows data sharing upon request or with approval/authorization and
