Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Rhode Island.
General Information
Rhode Island's IIS is the Rhode Island Child and Adult Immunization Registry (RICAIR), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.
The jurisdiction is authorized by law to operate RICAIR. For more information, please see Rhode Island General Laws § 23-1-18.
Child Consent to Participate
For children, RICAIR requires implicit consent with opt-out by law. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

For adults, RICAIR requires implicit consent with opt-out by law. Adults are included in the IIS unless they choose to opt out.

Provider Reporting
Rhode Island does have a mandate for vaccination providers to report immunizations to RICAIR. The reporting mandate:
- Applies only to a subset of vaccine recipients,
- Applies only to certain providers, and
- Applies only to specific immunizations.

Rhode Island does have a policy that regulates sharing data from RICAIR with other public health entities. The operational data sharing policy:
- Requires a data use/sharing agreement or memorandum of understanding and
- Allows data to be shared upon request or with approval/authorization.
