What to know
Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in New Jersey.
General Information
New Jersey's IIS is called NJIIS, and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.
The jurisdiction is authorized by law to operate NJIIS. For more information, please see New Jersey Statutes 26:4-134.
Child Consent to Participate
For children born on or after January 1, 1998, NJIIS requires implicit consent with opt-out by law. These children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

Adult Consent to Participate
For adults born before January 1, 1998, NJIIS requires explicit consent with opt-in by law. These adults must give explicit consent, or opt in, to participate in the IIS.

Provider Reporting
New Jersey does have a mandate for vaccination providers to report immunizations to NJIIS. The reporting mandate:
- Applies only to a subset of vaccine recipients.

New Jersey does have a policy that regulates sharing data from NJIIS with other public health entities. The data sharing policy:
- Requires that the data are only used for public health or other specific purposes and
- Requires protecting the data's confidentiality.
