Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Florida.
General Information
Florida's IIS is called Florida (FL) SHOTS, and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.
The jurisdiction is authorized by law to operate FL SHOTS. For more information, please see Florida Statutes Annotated § 381.003(1)(e).
Child Consent to Participate
For children aged 0 to 17 years, FL SHOTS requires implicit consent with opt-out by law. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

Adult Consent to Participate
For adults who are college or university students aged 18 to 23 years, FL SHOTS requires implicit consent with opt-out. These adults are included in the IIS unless they choose to opt out.

Provider Reporting
Florida does have a mandate for vaccination providers to report immunizations to FL SHOTS. The reporting mandate:
- Applies only to a subset of vaccine recipients,
- Applies only to certain providers, and
- Applies only to specific immunizations.
