What to know

Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Chicago.

General Information

Chicago's IIS is the Illinois Comprehensive Automated Immunization Registry Exchange (I-CARE), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

Child Consent to Participate

For children, I-CARE requires implicit consent with opt-out by law. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

For adults, I-CARE requires implicit consent with opt-out by law. Adults are included in the IIS unless they choose to opt out.

Provider Reporting

Chicago does have a mandate for vaccination providers to report immunizations to I-CARE. The reporting mandate:

  • Applies only to specific immunizations.
