This webinar helps participants understand the importance of EHRs in supporting type 2 diabetes prevention and diabetes management and is based on CDC guidance. Information about ways to support health care providers using EHRs to screen for, diagnose, and manage prediabetes and diabetes is provided. Examples from the field and additional resources are included.
Increasing referrals to diabetes prevention and management programs
A health care provider referral can be an important predictor of enrollment in evidence-based prevention and management programs. Webinar participants will:
- Learn the benefits of increasing health system referrals to these programs.
- Understand different referral strategies.
- Identify strategies that increase referrals to programs such as the National Diabetes Prevention Program (National DPP) and diabetes self-management education and support.
