Prevalence of Diagnosed Heart Disease Among Medicare Beneficiaries Enrolled in Fee-for-Service Medicare, 2015, by county. This map shows the concentrations of counties with the highest prevalence for heart disease – meaning the top quintile – are located primarily in Louisiana, Alabama, Tennessee, Oklahoma, West Virginia, Ohio, Pennsylvania, and New York, with pockets located along the eastern seaboard, Michigan, Arkansas, Arkansas, Missouri, and Nebraska.

Prevalence of diagnosed heart disease for 2020 for all Medicare Beneficiaries by County. The map shows that the concentrations of counties with the highest diagnoses of heart disease – meaning the top quintile – are located primarily in Florida, Georgia, Alabama, Mississippi, Louisiana, Texas, Arkansas, Oklahoma, Tennessee, Kentucky, and West Virginia. Pockets of high-rate counties also were found in Puerto Rico, North Carolina, Pennsylvania, Ohio, Indiana, Michigan, Illinois, Missouri, New York, Maine, Nevada, and California.
