Addressing Conditions to Improve Population Health

At a glance

The National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) invests in efforts to implement policy, systems, and environmental change interventions addressing social determinants of health (SDOH) in communities. The Addressing Conditions to Improve Population Health (ACTion) funding mechanism is one of its key investments.

Intended outcomes

Specific outcomes to be achieved through this award include:

  • Changes in group and organizational behavior and norms.
  • Adoption of new policies and practices.
  • Changes in availability and accessibility of health care and community services.

Funding

Five recipients will complete 3-year (2023-2026) ACTion projects focused on implementing established plans. The plans include policy, systems and environmental interventions in at least one of four NCCDPHP SDOH domains.
